 Resumo:

É crescente o índice de acidentes no trabalho nos serviços de saúde. Os estudos desenvolvidos nesta área são ainda incipientes e mais direcionados ao estabelecimento de nexos causais entre os fatores de risco. Na UTI, tais estudos são ainda mais escassos. Esta pesquisa tem o intuito de ir além dos aspectos biológicos, buscando envolver os aspectos mais subjetivos os quais, em inter-relação com a dinâmica do ambiente social dos profissionais, contribuem tanto para a produção de vivências de bem-estar como para as vivências de mal-estar. Preocupa-se ainda, com a com a necessidade de produzir conhecimento subsidiário para a implementação de políticas públicas que viabilizem a melhoria da saúde da população, tanto do servidor como do usuário. Este estudo de caso teve como objetivo investigar o Custo Humano da Atividade, as Estratégias de Mediação utilizadas pelos servidores de uma Unidade de Terapia Intensiva (UTI) de um Hospital Público e suas implicações para as vivências de bem-estar e mal-estar no trabalho. Os participantes foram todos os servidores da UTI que tinham contato direto e intenso com pacientes: sete médicos, sete enfermeiros e trinta técnicos de enfermagem. A metodologia utilizada na pesquisa foi Análise Ergonômica do Trabalho, com a utilização de observação livre, de análise documental, de entrevista do tipo semi-estruturada e de aplicação de escalas psicométricas (ITRA). Três questões nortearam a démarche de investigação: Como se caracteriza o Contexto de Serviços da UTI do Hospital? Como se configuram os Custos físico, cognitivo e afetivo nas atividades desenvolvidas na UTI do Hospital? Quais são as estratégias de mediação adotadas pelos trabalhadores no desenvolvimento do trabalho? Os principais resultados demonstraram um contexto de serviço precário (falta de material, falta de suporte organizacional, falhas na supervisão das atividades dos técnicos, dificuldades no relacionamento entre colegas e com o paciente) e com índices críticos no Custo Humano do Trabalho nas suas três dimensões: física (dores no corpo e cansaço), cognitiva (processamento de vários procedimentos ao mesmo tempo) e afetiva (constante exposição à dor, ao sofrimento e à morte). Desta forma, os servidores desenvolviam estratégias de mediação que nem sempre eram eficazes (face à situações nas quais não há mais o que fazer, dizer que ainda há esperança), dando origem a produção de vivências de mal-estar para os profissionais desta UTI. 

Abstract:

The index of work accidents in the health services is increasing. The studies developed in this area still incipient and more are directed to the establishment of causal nexuses among the risk factors. In the UTI, these studies are still more scarce. This research has intention to go beyond the biological aspects, it tries to involve the aspects more subjective those in interrelation with the social environment dynamics of the professionals, they contribute such a way for the production of experiences of wellbeing as for the malaise experiences. This study brings the preocupation with the one with the necessity to produce knowledge subsidiary for the implementation of public politics that makes possible the improvement of the population health, as the server as the user. This study of case had as objective to investigate the Human Cost of the Activity, the Strategies of Mediation used by the servers of a Unit Therapy Intensive (UTI) of a Public Hospital and the implications these for the experiences of well-being and malaise in the work. The participants were all servers of the UTI and had direct and intense contact with patients: seven doctors, seven nurses and thirty technician of nursing. The methodology used in this research was Ergonomic Analysis of the Work, with the use of free observation, of documentary analysis, of half-structuralized interview, and application of psychometrics scales (ITRA). Three questions guided démarche investigation: How is characterized the Context of Services of the UTI from Hospital? How the physical, affective and cognitive Costs are configured in activities developed in the UTI from Hospital? Which the strategies of mediation are adopted by the workers in the development of the work. The main results demonstrated a context of precarious service (material lack, lack of organizational support, imperfections in the supervision of the activities of the technician, difficulties in the relationship among colleagues and with the patient) with critical index in the Work Human Cost of the in three dimensions: physics (pains in the body and fatigue), cognitive (processing of some procedures at the same time) and affective (constant exposition to pain, to suffering and to the death). In these way, the servers developed mediation strategies that aren’t efficient all time (face to the situations in which it doesn’t have more what to do, to say that still it has hope); these situations have given beginning at the production of experiences of malaise for the professionals of this UTI.
